Job Description
Empower youth transitions as a Youth Support Services Facilitator with Sandy Bay Child and Family Services. Focus on creating supportive environments for young adults to thrive as they step into independence.
In this role, you will assist young adults of the Sandy Bay Ojibway First Nation in achieving their personal and educational goals. Your responsibilities include conducting eligibility assessments, developing tailored service plans, and guiding participants in accessing vital community resources. You'll also engage in outreach activities and foster cultural and recreational participation to enhance well-being.
Key Responsibilities:
• Conduct assessments to evaluate participant needs
• Create and implement individualized service plans
• Provide mentorship and case management support
