Job Description

Position Overview

Under the general direction of the Manager or designate, the Utilization Facilitator is responsible for

  • Facilitating and coordinating the movement of patients through the admission, transfer, and discharge process as required.
  • Liaising between internal and external agencies on matters related to patient movement.
  • Participating in the development of guidelines and educational strategies related to patient admission, transfer, and discharge.
  • Participating in decision-making that results in an effective admission process and the optimal utilization of facility resources.

    • Experience

  • Minimum 4 years recent nursing experience.
  • Recent acute clinical experience preferred
  • Demonstrated ability to function in a pivotal role, as facilitator in the patient admission/discharge/transfer process between patient, family, physician, Clinical Manager, members of the multidisciplinary team...
