Job Purpose

The Social Worker works collaboratively with a multidisciplinary team and partners with government organizations, non‑profit organizations, the private sector, and community groups to identify and improve social determinants of health that impact the overall wellness of individuals, families, and the community. The role uses a population health approach through individual supportive counseling, group work, education, health promotion, advocacy, chronic disease management, and linking people to community resources, helping them navigate systems. The work is client and family centered and guided by community health needs assessments as well as requests and referrals from clients, families and the community.

Qualifications

• Registered and a member in good standing with the Recent Brunswick Association of Social Workers.
• Non‑Violent Crisis Intervention may be required or obtained within 1 year.
