Job Description
Summary
Supports the mission of the hospital by aiming to reduce hospital readmissions through patient discharge optimization and follow-up. Performs clinical duties and engages in patient care to assist with medication safety, increase patient medication and disease state understanding, and encourages provider follow-up and continuity of care following hospital discharge.
Minimum Qualifications
Experience is PGY‑1 residency; PGY‑2 residency.
Education
- Bachelors Degree
- Doctoral Degree
- Pharmacy specialization
Education Equivalent Experience
- Bachelors – Pharmacy (required)
- Doctorate – Pharmacy (preferred)
Credentials
- Pharmacist (essential)
- Pharmacist – State of Florida (equivalent required)
- ASHP (Pharmacists) (preferred)
