Coverage Dates:
- Starting ASAP (7a) - ongoing for at least 2 months
- After initial 2-month assignment, facility anticipates sporadic future coverage needs
Schedule & Call Details:
- 24-hour call coverage
- Shift: 7a-7a
Reason for Coverage:
- Provider experienced a medical emergency and is unable to cover
Practice Setting:
- Full-service acute care hospital
- Hospital-based practice
Patient & Practice Details:
- Adult patient population: 100%
- Hospital-based caseload: 100%
- Physicians in practice: 1
- Mid-level providers: 2
