The Social Care Community Engagement Team is at the forefront of implementing Human Learning Systems approach Norfolk's approach to delivering a new model of social care. This tried and tested method focuses on strengthening our front door and encouraging social care professionals to have meaningful conversations with individuals to create better outcomes. It moves away from process-driven 'hand-offs' and promotes connecting with local networks and community support. By fostering these genuine interactions, we aim to enhance the quality of care and support available in our neighbourhoods and communities.
