**Shift:** Day Shift, Monday - Friday (8:00 am - 4:30 pm, on call from 4:30pm to 8am one week out of the month)

**Position Purpose**
The Social Worker monitors the patient’s progress throughout hospitalization and documents relevant findings with regard to Social Services and discharge planning in a timely manner. Responsibilities as follows: Establish accountability for data-driven quality care, service and financial outcomes related to patient transitions of care.
