(3) Full-time openings (Permanent)- Social Workers (Float)
M-F, 8hrs with weekend rotation (every 3rd/4th weekend).
Requirements:

Responsibilities: Early Identification, high-risk screening, and readmission screening of all patients. Services include: adjustments to illness counseling, safety assessments, discharge planning, identification of obstacles and barriers to discharge through proactive screening and assessments, and coordination with the multi-disciplinary team.

- 1+ years of discharge planning experience in an inpatient setting
- Must have EPIC experience
