As a Social Worker you will work closely with a variety of teams and agencies—including health colleagues, community and prevention teams, voluntary and independent sector partners, and others—to support people in a person‑centred way, utilising their unique strengths and celebrating individuality. Through strength‑based assessments and interventions, you will help people with care and support needs and their carers to achieve desired outcomes with a focus on wellbeing, and you will be expected to think creatively when looking for solutions for individual needs.
