Our well-established team includes Social Care Practitioners, Occupational Therapists, Occupational Therapy Assistants, Reablement Officers, Reablement Supervisors, and Reablement Assistants, all dedicated to achieving the best outcomes for our customers on their Reablement journey.

This is a fantastic opportunity to become a Social Care Practitioner in our county-wide Reablement Service. In this role, you will assess customers in their own homes, following a therapy-led care package provided by our team. We pride ourselves on a holistic approach that embraces multi-professional skills, empowering customer choice and control.
