Responsibilities
- Manage clinical caseload and support palliative rehabilitation practice (e.g. breathlessness management, prescription of equipment, collaborative goal setting) across the various care settings.
- Deliver evidence‑based individualised care through holistic comprehensive assessment, treatment and evaluation of care.
- Work collaboratively with multi‑disciplinary colleagues to meet patient needs.
- Participate in service review to drive service improvement.
- Contribute to the implementation and monitoring of adherence to relevant clinical guidelines.
- Act as a resource and support to other team members and community professionals.
- Take initiative in pursuing life‑long learning and personal development through reflective practice and engage actively with learning and development opportunities.
