About This Role
This position provides support to the Sisters of the Holy Names of Jesus and Mary (SNJM) residing in the Windsor, Ontario region. Consults and advises regarding healthcare needs and services and performs all essential functions of care coordination. Under the supervision of the Director of Healthcare Services, coordinates daily support services as needed by individual Sisters and helps to enhance each Sister’s understanding of her own medical needs.
Essential Duties and Responsibilities
- Assists and advises individual Sisters who have requested assistance or who have been identified as needing assistance with healthcare and related services to support their independence and individual preferences.
- Performs all essential functions of care coordination, which include assessment, development of health and related needs-based plan, implementation, monitoring, evaluation, and adjustment.
