Registered Nurse (RN) – Hospice
Position Summary
The Registered Nurse (RN) provides nursing care to hospice patients in their place of residence and coordinates care with the Interdisciplinary Team (IDT), physicians, patients, families, and referral sources.
The RN is responsible for assessing patient needs, implementing the plan of care, and ensuring quality end-of-life care.
Responsibilities
Perform initial and ongoing patient assessments
Develop, implement, and update individualized plans of care
Coordinate care with the IDT, physicians, patients, and families
Provide patient and caregiver education regarding disease process, medications, end-of-life care, and safety
