Job Description
Social Worker role based at Bukit Batok and Buangkok locations. You will develop individualized care plans for residents and execute them, monitoring progress and adjusting as needed.
You will liaise with care teams, document progress, and ensure adherence to care standards. You will develop and implement programmes and activities to meet residents' care needs, and prepare six-monthly reviews and monthly interview records for about 50 residents. Training is provided.
