Will assist the patient of Breaking Free foundation in their day by day activities

• Needs Assessment: Social workers interview clients to understand their circumstances, identify needs, strengths, and challenges, and determine achievable goals.
• Intervention & Support: They provide emotional and practical support, counsel clients, and help them develop coping mechanisms to address life changes, illness, or other crises.
• Resource Connection: Social workers connect clients with community resources, health services, housing assistance, food programs, and other benefits to improve their well-being.
• Crisis Intervention: They are trained to respond to emergencies such as child abuse, domestic violence, mental health crises, and trauma, intervening to ensure safety.
