What Youāll Do:
ā¢ Conduct move-in, 3-day, semi-annual, and change-of-condition assessments
ā¢ Perform monthly wellness visits and monitor health trends
ā¢ Follow up on falls, incidents, and hospital returns
ā¢ Transcribe physician orders and ensure accurate MAR/TAR documentation
ā¢ Complete medication audits and oversee medication program compliance
ā¢ Partner with families, physicians, and the Resident Care Director
ā¢ Promote infection control and survey readiness
