Job Description
Enhance community health as a Registered Nurse with Mosaic Primary Care Network. This role focuses on patient assessment, chronic disease management, and health coaching within a collaborative team.
Reporting to the Supervisor of Patient Access, the Primary Care Nurse will provide comprehensive nursing services at our clinics. You will evaluate patients' primary and complex health needs, implement care plans, and engage in health promotion activities. This is an opportunity to work with interdisciplinary teams to improve patient outcomes and foster wellness in the Calgary communities.
