Job Description
KEY FUNCTION
Assist in establishing and maintaining a safe and therapeutic environment through the provision of therapeutic intervention and engagement with the patients and the inter‑professional team. Provide care to patients in acute care transitioning to the Reactivation Care Centre.
REPORTING
Under the general direction of the Clinical Manager, Acute and Reactivation Care Unit.
DUTIES
- Implement therapeutic, geriatric‑focused care of patients with the goal of identifying restorative potential.
- Work collaboratively with the clinical team to best meet the unit or patient’s needs.
- Implement and guide patients through recommended treatment and activity programs both individually and in groups (e.g. assist with preparing patients for treatment, treatment modalities, activities of daily living, walking programs, transfers, and communication skills).
