Programme Coordinator (Geriatric Medicine)

We are looking for a compassionate, proactive, and organised individual to support the delivery of comprehensive geriatric assessment services. In this role, you will work closely with Geriatricians, nurses, allied health professionals, patients, caregivers, and community partners to ensure holistic and coordinated care for older adults.

Geriatric Assessment

• Conduct outpatient geriatric cognitive assessments in accordance with established clinical protocols.
• Gather and document relevant patient information to support clinical assessment and care planning.
• Provide patients and caregivers with information on cognitive health and available support services.

Care Coordination

• Conduct pre-appointment calls to patients and caregivers to facilitate completion of assessment questionnaires and prepare them for appointments.
