Position Summary
The Post-Acute Care Physician provides high-quality medical care to residents across assigned skilled nursing facilities. This role is responsible for managing a defined patient census, supporting facility leadership, and improving clinical outcomes, documentation, and operational performance.
The Post-Acute Care Physician provides high-quality medical care to residents across assigned skilled nursing facilities. This role is responsible for managing a defined patient census, supporting facility leadership, and improving clinical outcomes, documentation, and operational performance.
The physician will provide care for post-acute rehabilitation, long-term care, and custodial residents, including admissions, follow-up visits, acute visits, regulatory visits, and chronic disease management. This position requires strong clinical judgment, effective communication skills, and the ability to work independently while collaborating with facility teams.
