Job Summary: We are seeking a compassionate and skilled Physical Therapist (PT) to join our interdisciplinary team in a Skilled Nursing Facility (SNF). The Physical Therapist is responsible for evaluating residents’ physical conditions, developing personalized treatment plans, and delivering rehabilitative care to help patients improve mobility, restore function, and achieve the highest possible level of independence. Key Responsibilities: Perform thorough assessments of patients’ physical abilities, movement limitations, and pain levels. Develop and implement individualized treatment plans based on evaluation findings, medical history, and patient goals. Deliver evidence-based physical therapy interventions to improve strength, coordination, balance, and mobility.
