**Physical Therapist- Home Health – HOME HEALTH OR WEST PORTLAND at Murray Building - Home Service West Portland**
**Schedule:** Day shift, Monday - Friday position plus 2 weekend rotation every 2 months
**Area Coverage:** Beaverton/Portland
**Job description:**
Manages and provides skilled physical therapy to patients/clients with a wide variety of diagnoses and disabilities in a home setting in accordance with a physician plan of treatment, taking into account individual patient special or age related needs. Requires skills in patient/client evaluation and treatment, as well as professional consultations to achieve maximum patient/client independence in accordance with the individuals functional level and potential.
