In this Case Manager position, you will collaborate directly with the Home and Community Care Manager to execute client care plans. Applicants with a Bachelor’s degree in Nursing, Social Work, or Occupational Therapy will be prioritized. You will also oversee the collection of information to ensure service provision aligns with client needs.
