Home Care is a multi-disciplinary team with RN, LPN, SW, RT, PT, OT, and Rehab Assistants who collaborate and conduct home visits to deliver services to clients with a wide variety of conditions, including complex health needs. As an Occupational Therapist I in the Rural South Home care team, you will act both as a Case Manager as well as a direct care provider, utilizing your strong skills in family-centered practice, coaching, assessing, organizing and communication. The Case Management duties include ongoing assessing and identifying the overall needs of the client, in collaboration with the family and other partners, and developing a care plan based on those needs. The care plan ensures appropriate services are provided, and the appropriate internal and external resources are accessed.
