We are seeking dynamic and passionate Occupational Therapist (OT) to join Elizz by SE Health, an innovative and growing community care service that is part of SE Health. In this role, you will provide high-quality, client-centered rehabilitation services to individuals in the community while supporting a new hospital-to-home care initiative designed to improve the transition from hospital to home.
Working within a collaborative and interdisciplinary care model, you will partner with a diverse care team to support clients in achieving their health and recovery goals. You will help develop and implement holistic care plans that address clients’ physical, cognitive, social, emotional, and spiritual needs.
