Job Description
Responsibilities
- Oversee monitoring of patient vitals, symptoms, and labs in collaboration with RNs and MDs
- Manage treatment plans and medications with emphasis on GDMT for Heart Failure, Hypertension, and Type 2 diabetes
- Respond to patient escalations identified via remote patient monitoring
- Lead regular virtual check-ins with patients to review labs and medication optimization
- Ensure exceptional healthcare interactions that prioritize patient well-being
- Support internal growth efforts to scale care delivery for chronic conditions
Requirements
- 5+ years experience treating chronic diseases (CHF, Hypertension, T2D)
- Experience managing CHF with GDMT
- Active ANCC or AANP certification
- Master’s Degree as a Nurse Practitioner
- Active Compact Multi-State RN license
- Active NP licensure in at least two of: TX, MI, NC, CA, PA, NJ, CT, AL <...
