Become an essential part of the La Ronge Health Centre as a Nurse B Clinician focused on patient case management. This full-time temporary role is pivotal for care coordination in healthcare.
Reporting directly to the Primary Health Nursing Coordinator, you will provide holistic healthcare solutions for patients at Pinehouse Health Centre. The RN Case Manager will engage in outreach and programming while emphasizing the management of communicable diseases. Effective communication and relationship building with patients, their families, and the healthcare team will be central to your role.
Key Responsibilities:
