Job Description
Job Title: Site Physician Assistant
Department: Clinical Operations
Reports To: Chief Medical Officer
Schedule: Monday–Friday, 8:00 AM – 5:00 PM (40 hours/week)
Position Summary
The Site Physician Assistant provides high-quality, patient-centered care within a Diagnostic Treatment Center (DTC). This role includes primary care delivery, care coordination, and clinical leadership within an interdisciplinary team, with a focus on serving frail and elderly populations.
Key Responsibilities
- Provide primary care and manage a panel of patients in clinic and home settings
- Lead and support care delivery across the interdisciplinary team
- Participate in care planning, case conferences, and utilization review
- Monitor and improve quality, cost, and outcomes of care
