Currently seeking a Nurse Practitioner to join a team full time at a Hospital at Home ProgramUnder the supervision of the Hospital at Home (HAH) Medical Director the nurse practitioner will perform/supervise services according to the policies, procedures, philosophy, and objectives of the department and hospital Virtual patient visits and/or in-person home visits Will share on-call responsibilities with the Physician/Medical DirectorResponsibilities include: Complete admission physical exams accurately, thoroughly, and in a timely manner utilizing appropriate growth and development parameters Assess patient/family psychosocial needs, educational needs, learning readiness, and barriers to learning during admission process.
