Brief Overview:
Under the direction of the Mobile Outreach Project Manager , the Mobile Outreach Support Worker is responsible for supporting the Health and Mental Health & Addictions department, conducting assessments, and identifying mental health needs, advocating, mentoring, and connecting with patients through support groups and social services.
