Job Description
**Amergis Healthcare is seeking an LPN In Home Case Manager to provide admissions assessments in the Northeast region of Boston. In this fulfilling role, you will provide community-based care services designed to help elderly and disabled individuals remain safely and comfortably in their homes.**
**Local Pay: $47/hour + mileage reimbursed at $0.72/mile**
**Location:** Home-based visits around Reading, Lowell, Lawrence and Salem area
**Qualifications:**
+ **Active, unrestricted MA nursing license**
+ **Minimum 2 years of recent experience working with elderly populations or individuals with disabilities**
+ **Valid driver’s license**
+ **Bilingual (English + another language) preferred**
+ **Experience in home health, case management, or community-based care preferred**
**Schedule:** **Monday–Friday, 9:00 AM–5:00 PM**
