Advanced Care Partners is seeking a Licensed Practical Nurse (LPN) to provide one-on-one care for medically fragile children and adults in home and school settings. This role offers the opportunity to deliver high-quality, individualized care in a more focused and meaningful environment.
Base Pay: $35/hour
Opportunity to earn up to $43/hour with applicable differentials:
- Trach: +$2/hour
- Vent: +$3/hour
- Nights (7pm–7am): +$2/hour
- Weekends: +$3/hour
- Holidays: +$3/hour
- Weekly pay
- Flexible scheduling options (full-time, part-time, PRN)
- Cases available in close proximity to your home
- Consistent patient assignments
