Location : Mt Airy, NC 27030
Facility : LTC/SNF
Shift : 7am - 7pm & 7pm - 7am ( Three days in a week that includes EOW)
Contract : 13 weeks

- Updated Resume
- Two Supervisor references along with the facility details
- BLS/CPR
- Immunization records (PPD, Flu and Physical) if updated
- Covid Vaccination Card
- State LPN license
- BLS/CPR preferred
- Min 1 -2 year of LPN experience in rehab or skilled nursing facility will be preferred .
- Duties include taking Vitals, assisting with ADL's, transportation, Medicine administration and reminders, Assisting RN's.
- Care/Rehab or Skilled Nursing Facility is preferred.
