Summary The LPN at the Loch Raven VA Medical Center serves as a member of the interdisciplinary health care team with selected patient and provides care to them on a regular and recurring basis. This is an Intermittent Position. Gathers additional data (face to face or via phone) related to patient needs for treatment determination of PACT.
