Job summary
This role is an exciting opportunity for a highly motivated and dedicated community clinician with experience of supporting learning, encouraging clinical curiosity and confidently capable of assessing and responding to patients with complex long-term conditions. Working as part of the Ageing well development team, you will be encouraged to help design and deliver evidence-based knowledge about frailty and ageing well, understanding the importance of functional fitness, managing disease and coaching conversations. Reflecting the growing demands and clinical complexity of our population in Cornwall, we have committed to developing the skills and versatility of our workforce. This will require the post holder to deliver clinical education, leadership and supervision to our growing and developing workforce, including supporting clinical practice development for our preceptees.
