Position Highlights

- Practice in Skilled Nursing Facilities (SNFs) and Assisted Living communities
- Build lasting relationships with patients and facility teams
- Work independently with a high level of clinical autonomy
- Base salary of $275,000
- Productivity incentives with earning potential of $350,000 $400,000+ annually through RVUs
- Board Certified or Board Eligible in Family Medicine , Internal Medicine , or Geriatric Medicine
- Comfortable practicing independently in post-acute care settings
- Strong interpersonal skills with a patient-centered approach
