Job Description
SCOPE OF THIS RFP
The State of New Hampshire needs an external evaluation partner to oversee their Rural Health Transformation Program (RHTP) — a CMS-funded statewide initiative. The vendor will evaluate five health programs running across New Hampshire, monitor performance data monthly, report findings to state leadership and CMS, and help course-correct when programs are not on track. This is an evaluation and advisory services contract — NOT a staffing placement contract. Contract value is up to $25 million through 2031.
POSITION 2 — Implementation Science Specialist
What they will do: Assess why health programs are or are not working using implementation science frameworks; support mid-course corrections
Minimum experience needed:
- 5+ years applying implementation science in healthcare or public health
- Must know at least one major framework: EPIS, CFIR, or PRISM
