Job Description
CHEO is seeking a Pediatric Care Coordinator to enhance care for children and youth. You will lead service coordination across home, school, and community settings in a hybrid model.
In this key role, you will perform assessments, create care plans, and facilitate smooth transitions from hospital care. A degree in Nursing, Occupational Therapy, or similar is essential, as well as strong communication skills to collaborate effectively with families and interdisciplinary teams.
Key Responsibilities:
• Conduct assessments and determine service eligibility
• Develop and implement comprehensive care plans
• Collaborate with CHEO teams for safe patient transitions
• Evaluate outcomes and adjust care as needed
