Shape the future of at-risk youth as a full-time Care Planner with the Ma Mawi Wi Chi Itata Centre. Support and manage caseloads for females and transgender youth aged 13-17 in a safe environment.
The Ma Mawi Wi Chi Itata Centre is looking for a Care Planner. This full-time role focuses on providing case management and support services for young individuals in a six-bed facility. You will develop healing plans, facilitate programs, and supervise residents to foster a nurturing environment.
Key Responsibilities:
