Job Description
,Ā administerĀ andĀ monitorĀ funding, such as Special ServicesĀ AtĀ Home,Ā CSN and Flex Funding,Ā andĀ assistĀ families/guardians with application and billing procedures, such as thoseĀ requiredĀ for Ministry funding or other programsĀ - ConsultĀ and collaborateĀ with colleagues,Ā clinical,Ā medical,Ā educationalĀ and other community services in conjunction with families/guardians to plan, coordinate, and evaluate support (such as transportation, medical appointments,respite care) using various communication methods. Provide and/orĀ facilitateĀ trainingĀ opportunities asĀ identifiedĀ through theĀ Personal PlanĀ Transitional AgedĀ Youth,Ā ServiceĀ and/or Care Plans. Schedule employees to ensure adequate coverage for after school programs, camps etc. Participate inĀ staffĀ and communityĀ meetings.
