Responsibilities
Reporting to the Director, Clinical Services, the Community Response Coordinator serves as part of the senior nursing team. The coordinator provides clinical support to the community palliative care services of the Victoria Hospice Society (VHS), including consultation with community, acute and long‑term care partners, patients and families, and outpatient and in‑home support for the Palliative Response Team.
