Full-time Home Health and Personal Care Aides; and Nursing Assistants, Orderlies, and Psychiatric Aides

Position Summary

Countryside Post Acute is seeking a full-time, onsite CNA to deliver routine daily nursing care aligned to each resident’s assessment and care plan on rotating shifts (days/evenings/overnights; weekends and holidays as needed). Pay range: $17–$20 per hour.

Key responsibilities include assisting with resident ADLs; obtaining and recording vital signs; documenting care in the EMR using authorized abbreviations; supporting admissions/transfers/discharges; providing catheter and perineal care; using lifting/transfer equipment (including Hoyer lifts); and promptly reporting condition changes, incidents, and safety concerns to the Charge Nurse.

Required qualifications/skills:
- Active Certified Nursing Assistant (CNA) certification
- Current CPR/BLS
- Working knowledge of infection control (OSHA/CDC bloodborne pathogens, TB protocols)
