Overview

Clinical Pharmacist Prescriber to join our GP surgery at Addison Road Medical Practice. Key role in delivering safe, effective medication use and optimizing patient care within a multidisciplinary team.

Responsibilities

• Prescribing and clinical triage via online, face-to-face and telephone, including management of minor illness and long-term condition (LTC) advice.
• Deliver safe clinical decisions, expert care and collaborate with all team members to drive innovation and patient satisfaction.
• Manage same‑day demand, working within the Triage Hub to assess and respond to patient requests.
• Use clinical expertise to streamline workload and provide administrative support for demand management.
• Conduct structured medication reviews and run long‑term condition clinics for conditions such as hypertension, diabetes, asthma and COPD.
