About the Role.
As a Community Health Nurse, you'll provide person-centred nursing care across a variety of community and home-based settings, supporting individuals and families to achieve the best possible health outcomes. Working both independently and as part of a multidisciplinary team, you'll assess client needs, coordinate care, deliver health education, and contribute to health promotion initiatives.
