Scope of Work
Conducts and records spiritual assessments of patients in a manner which is useful to departmental colleagues and members of interdisciplinary healthcare teams.
Provides pastoral care, counsel, emotional support and spiritual/religious rituals—as requested and appropriate--to patients, families and staff. Otherwise, the chaplain identifies additional, available resources and assists with referrals. Carefully documents interventions provided.
Collaborates closely with departmental colleagues around shared responsibilities, timely referrals, accurate charting and logging, flexibility in helping with unexpected involvements during a shift, making patient hand-offs and follow-ups, taking pager coverage, and offering mutual support.
Collaborates with members of the interdisciplinary health care team in identifying holistic approaches to patient care needs, and in facilitating progress towards resolution of these needs...
