As a Designated Nursing Assistant / Certified Nursing Assistant (DNA/CNA), you provide direct, hands-on care that supports residentsā clinical, personal, nutritional, and restorative needs in our skilled nursing community.
What Youāll Do:
ā¢ Deliver care based on Individualized Service Plans (ISP)
ā¢ Assist with ADLs including bathing, dressing, mobility, dining, and toileting
ā¢ Obtain and report vital signs, intake, and changes in condition
ā¢ Partner with licensed nurses on restorative and care planning programs
ā¢ Support fall prevention, skin care, and quality improvement initiatives
ā¢ Maintain accurate documentation and restorative records
ā¢ Respond promptly to resident, family, and visitor concerns
