Job Description
Overview
The Case Manager provides comprehensive care management services to ALP and ACF residents. This includes, but is not limited to psychosocial evaluation, evaluations for admission, residentβs rights and advocacy, group work and family counseling as appropriate. The Case Manager is a key member of the interdisciplinary care team and is responsible for ensuring maximum quality of life for an assigned group of residents or clients.
This position will be located onsite at our Syracuse, NY location. This is not a hybrid or remote opportunity.
