Job Description
At Bethanie, our work supports older Australians to live well,
maintain their independence and continue doing the things that
matter most to them. As a Case Manager in our Home Care team,
you'll play a key role in partnering with clients to achieve their
goals and remain living safely and confidently at home.
Working as part of a supportive multidisciplinary team, you'll
collaborate with Clinical Nurses, Allied Health professionals,
Support Workers and fellow Case Managers to deliver personalised
care solutions that promote wellbeing, choice and dignity.
About the role
As a Case Manager, you'll coordinate and manage Home Care
services for a portfolio of clients, taking a holistic wellness and
reablement approach to support independence, health and quality of
life.
