Title of Job: Case Manager Date: June 24, 2026 Closing Date: July 8, 2026 Location: Toronto, ON - Back to Home - East Toronto Compensation: $57,007 - $71,647 Annually Prorated Contract Duration: 6-Month Contract Work Type: Onsite Job ID: 10871615 Existing or New Position: Existing Job Summary: As a Case Manager you will work independently or be part of a team that co-ordinates a support network and links clients living with mental illnesses, addictions and physical health issues.
The position attempts to empower the client, thereby increasing their independence, dignity and selfrespect in a safe and secure environment.
Direct service and support will be provided to clients in areas such as daily living, practical assistance, crisis intervention, linkages to primary care and a range of other services to support the individual’s needs.
