Developmental Disabilities Case Management  Job   Description      The Case Manager Family Consultant is responsible for the up keep of each Individual’s chart, which houses complete records of the individual obtained at the time of referral and admission; to include, documentation of the work performed on the individual while in placement, and the charting of the Individual’s progress while serving as the provider.  ·        Provide written quarterly service plan reports on all assigned cases. ·        Participate in agency on-call rotation.
